Patient Rights and Responsibilities

M.S. Shook Student Health Service recognizes the rights of expression, decision, actions, and personal dignity. It is the responsibility of M.S. Shook Student Health Service to assure that these rights are preserved for patients.

M.S. Shook Student Health Service expects behavior on the part of patients, relatives, and friends to be reasonable and responsible. Behaviors are to be reflective of active participation in their own care and of adherence to clinic rules and regulations.

As a patient at M.S. Shook Student Health Service, your rights include:
  • The right to be treated with respect, consideration, and dignity.
  • The right to information regarding the scope and availability of services.
  • The right to know who is counseling, caring for, and treating you.
  • The right to accurate information, to the extent known, concerning diagnosis, treatment, and prognosis of a health-related condition, in terms that you can understand.
  • The right to receive the information necessary from your clinician to enable your consent prior to any procedure or treatment
  • The right to a second opinion regarding diagnosis or treatment.
  • The right to a consistent standard of care.
  • The right to participate in decisions that are made regarding your health care and treatment.
  • The right to be informed of any research aspect of your care and to refuse to participate.
  • The right to expect that your medical records are confidential and require your written authorization for release except as required by law.
  • The right to information about continuing health care requirements, such as return visits and required medications.
  • The right to register a complaint regarding the quality of care provided, quality of service delivered, or any other valid concern. Concerns may be directed to Beth Booth at 828-262-3100.
  • The right to information regarding fees for service, especially those which may involve additional charges.
  • The right to appropriate, timely, and empathetic care with a possible abuse or neglect victim.
As an active participant in your care, you also have the following responsibilities:
  • To provide complete and accurate information to the best of your ability about your health, any medications, including over-the-counter products and dietary supplements, and any allergies or sensitivities.
  • For your continued care, follow the treatment plan prescribed. Provide a responsible adult to transport you home and remain with you if required by your provider.
  • To inform your provider about any living will, medical power of attorney, or other directives that could affect your care.
  • To accept personal financial responsibility for any charges not covered by your insurance.
  • To conduct yourself in a manner that shows courtesy and respect to health care personnel and other patients.
  • To refrain from lending your student identification to others and not to give others medication prescribed for you.
  • To keep your appointments.  If you cannot keep an appointment, please call ahead to cancel or reschedule.
  • To be informed about your current healthcare needs, and the offerings of the Student Health Service.
