Immunizations
Before attending classes, all Appalachian State University students must submit a "Report of Medical History and Immunization Record Form" as required by University Policy and North Carolina law.

The Immunization and Medical History Form is available here on-line or at the Health Service. Each of the three sections is necessary to have a complete set.

Vaccine and Medication Price List
Health Services offers the vaccines listed below. Students will be informed beforehand of charges for vaccinations and medications that are recommended.
- Havrix (Hepatitis A): $20 each, 2 shots required; 1 now & 1 in 6-12 months
- Engerix B (Hepatitis B): $30 each - 3 shots required; 1 now, 1 in one month, & 1 six months from the first.
- Varivax (Chickenpox): (check for price); 1 now & 1 in 4-6 weeks
- Typhoid Vi Vaccine (injectable): $40
- Vivotif (live oral) Typhoid Vaccine: $30
- Meningococcal Vaccine: Menomune and Menactra: $95
- MMR: $50
- Tetanus Diphtheria: $20 (Tdap: $35)
- Tb Test: $5 (tests given Mondays, Tuesdays, and Fridays)
- Gardasil: $125 per dose (three doses required)
Anti-malarials
- Aralen 500 mgm (Chloroquine): $2 per tablet, A minimum of 6 tablets
(1 at least 1 week before travel, 1 each week of travel stay, and 1 each week for 4 weeks after returning) - Lariam 250 mgm (Mefloquine): $6 per tablet
- Malarone: $6 per tablet
