Required Forms for Students
At App State, we prioritize the health and safety of everyone on campus. New students must complete the following steps:
Immunization Record: Submit your immunization record in MedPortal before attending classes. Paper copies are also accepted by mail, in person, or via fax.
Refer to the current immunization requirements set by university policy and North Carolina law here. (International students must provide records in English.)
Health History Form: Complete the Health History form in MedPortal before your visit to the Student Health Clinic. You will receive a notification to complete this form before your first appointment.
