Required Forms for Students
- MedPortal Welcome Packet
- Immunization Requirements, Meningitis Information Sheet, Privacy Statement
The following forms are required and are filled out online via MedPortal (opens in a new tab):
- Immunization Submission (opens in a new tab)
- Medication/Allergy Submission (opens in a new tab)
- Student Under 18 Authorization to Treat(opens in a new tab)
Health History forms are not longer required before enrolling as a student. A notification will be sent to the student to complete the form before their first appointment.
