Welcome to App State! We're excited to have you join the Mountaineer Community. The Mary S. Shook Student Health Service is here to support your health needs.
Important Dates:
- Upon class registration: MedPortal opens for accessing health services.
- July 1st: Deadline to submit immunization records, health history form, and student insurance waiver/enrollment via MedPortal.
Required Enrollment Steps:
- Submit Immunization Records: Upload your immunization records through MedPortal, fax to 828-262-6958, mail, or email to [email protected].
- Complete Health History Form: Not required before enrollment; you'll be notified to complete it before your first appointment.
- Select Health Insurance Option: Enroll in or waive the mandatory health insurance each semester. You'll be charged for the Student Health Insurance Plan upon class registration; provide proof of other insurance to reverse the charge.
