Pre-Visit Forms
- Health History, Immunization, and Travel Form
  - Students will receive a notification of pre-visit forms needed for their scheduled appointment at the time the appointment is made.
Immunization Management
- Upload of immunization records and self-entry of immunizations

- Allows two-way communication between our Providers and you

- All educational handouts will be delivered to your MedPortal account

Accessing Your Immunization Records

To access your immunization records within MedPortal by following these steps:

Log into MedPortal (opens in a new tab)
Select the 'Immunizations' tab
- This will allow you to view all immunizations that you have submitted to App State along with all vaccinations administered by the App State Immunization Clinic
Click 'Print' if you would like to print your records
